Our Chart Pages and Worksheets use the Seven Steps system to help your staff members find a patient's emergency contact information or identify a John/Jane Doe quickly and easily even in the most challenging situations.  
 
Designed to work with your current charting system, the chart pages can be used to input patient information right on the computer, giving you a hard copy that can be printed and put into the patient's chart, or be scanned and saved as part of a computerized patient record.   The worksheets give you even more flexibility, providing an open design where information can be inputted via computer, typewriter or handwritten information noted on the fly.  They can also be easily customized with your hospital's logo.
